Customer Agreement
The Service Agreement is entered into by and between Pottawatomie Telephone Company and customer. The terms of the Agreement are as follows:
SERVICES PROVIDED: Pottawatomie Telephone Company shall provide High Speed Internet service and/or Digital TV service, including free installation if required, between customer’s premises and the designated Pottawatomie Telephone Company wire center.

*Premium Video services are available upon request and may not be part of the HSI contract. Please inquire with your Pottawatomie Telephone Company service representative regarding premium service packages and rates.
RATES AND CHARGES: Pottawatomie Telephone’s High Speed Internet Service/Digital TV Service shall be billed at the low monthly rate of $ (HSI/DTV rate shall be constant for the term of the agreement. Monthly billed amount may vary with customer request of premium services). Rates may not include modems, firewalls, Network Interface Cards (NIC), routers, set top boxes, remote controls, or piggy back equipment. Prices do not include government taxes and other fees.
BILLING FOR SERVICES: Customer acknowledges and agrees that billing for High Speed Internet service/Digital TV service will be conveniently billed on customer’s monthly telephone bill rendered by Pottawatomie Telephone Company. Customer further acknowledges and agrees that failure to pay all monthly amounts when due, including regulated and non-regulated services may result in disconnection of local telephone service as well as High Speed Internet/Digital TV service.
Customer understands that use of Pottawatomie Telephone Company’s Internet service is subject to our Acceptable Use Policy that can be found at www.potelco.com, or you may request a copy by contacting the Pottawatomie Telephone Company at 405-997-5201.
By signing the Agreement, Customer acknowledges and agrees to abide by the terms herein.
TERM OF AGREEMENT: The Agreement shall be effective on XX/XX/XXXX and shall continue for a period of one (1) year. If the customer terminates the agreement prior to the end of the agreement term, a termination charge of $185.00 will be due and payable. Also, any unreturned premise equipment (i.e. set top box &/or router) should be returned to Pottawatomie Telephone Company.
